What You'll Do
- Perform physical therapy evaluations, treatments, and ongoing assessments in accordance with physician-approved plans of care.
- Complete initial and comprehensive assessments (including OASIS) within 48 hours of referral or as scheduled.
- Develop, implement, and update individualized care plans based on patient needs and agency guidelines.
- Document evaluations, treatments, and outcomes accurately and submit within required timelines.
- Communicate patient status, care plans, and changes to physicians, case managers, and care teams.
- Report schedule changes and physician orders to the office promptly.
- Provide patient and caregiver education tailored to individual needs, environment, and learning abilities.
- Assess learning needs, deliver instruction, and evaluate effectiveness of teaching.
- Participate in care conferences, interdisciplinary collaboration, and discharge planning.
- Support quality improvement initiatives, including use of OASIS and outcome data to enhance patient care.
- Collaborate with the team to ensure consistent and accurate data collection.
- Maintain a safe environment by following infection control and safety protocols.
- Identify and address safety risks for patients and staff; ensure proper equipment use.
- Supervise and support Physical Therapist Assistants and home health aides per agency policy.
- Conduct performance reviews and provide feedback as assigned.
- Comply with all agency policies, professional standards, and state/federal regulations.
- Participate in required meetings, training, and ongoing professional development.
- Demonstrate proper use of therapy equipment, including ultrasound, as appropriate.
- Perform additional duties as assigned.
What You Bring
- Graduate of an accredited Physical Therapy program (APTA-approved or equivalent).
- Current Physical Therapist license in the state(s) of practice.
- Minimum of 2 years of healthcare experience; home care experience preferred.
- Current CPR certification required.
- Strong verbal and written communication skills with effective interpersonal abilities.
- Ability to work independently with minimal supervision.
- Valid driver’s license with reliable, insured transportation and a good driving record.
