One Home Health is currently looking for a motivated and caring Physical Therapist Assistant (PTA) to support our home health patients on a part-time basis. Primary Responsibilities
In this role, you will:
- Provide prescribed physical therapy treatments and patient education under the direction of the supervising Physical Therapist and according to the physician-approved plan of care.
- Monitor patient progress and communicate changes in condition, treatment response, or scheduling needs to the appropriate team members.
- Maintain timely and accurate clinical records that meet agency, state, and federal documentation requirements.
- Collaborate with nurses, therapists, physicians, and other healthcare professionals to promote positive patient outcomes.
- Assist with care coordination and participate in case conferences or interdisciplinary team meetings when requested.
- Follow established infection control practices, safety procedures, and equipment usage guidelines during home visits.
- Protect patient confidentiality and comply with all HIPAA and privacy standards.
- Adhere to agency protocols, regulatory requirements, and professional practice standards.
- Participate in educational programs, staff meetings, and ongoing quality improvement initiatives.
- Carry out additional duties related to patient care and agency operations as assigned.
Candidates should meet the following requirements:
- Current, unrestricted Physical Therapist Assistant license in the state of Illinois.
- Graduate of an accredited PTA educational program recognized by the American Physical Therapy Association (APTA).
- Minimum of six months of PTA experience in a healthcare setting preferred.
- Strong verbal communication, interpersonal, and clinical documentation skills.
- Reliable transportation with a valid driver's license and current automobile insurance.
- Ability and willingness to travel throughout the assigned coverage area to provide in-home patient care.
