Certified Occupational Therapy Assistant (OTA/COTA)

We are seeking a Part-Time/PRN Occupational Therapy Assistant (OTA/COTA) to join our home health team in Lake County, IN. This role is ideal for clinicians looking for Schedule flexibility, supplemental income, or a better work-life balance while making a real impact in patients’ homes.

• Provide in-home OT services under the direction of a licensed Occupational Therapist
• Follow and carry out established plans of care
• Educate patients and caregivers on safety, ADLs, and adaptive techniques
• Monitor patient progress and report changes to the supervising OT or nursing team
• Complete accurate, timely documentation in accordance with agency guidelines
• Participate in interdisciplinary collaboration as needed
• Promote patient safety, infection control, and use of proper equipment
• Maintain confidentiality and compliance with agency, state, federal, and ACHC standards

• Licensed Occupational Therapy Assistant (OTA/COTA)
• Home Health experience preferred, but not required
• Strong communication and documentation skills
• ability to work independently in a home-based setting

• Physical ability to perform in-home visits
