We are seeking a Part-Time/PRN Registered Nurse (RN) to provide high-quality, in-home care to patients across far South and Western suburbs of Chicago, specifically Aurora, Naperville, Plainfield, Bolingbrook, Minooka, Channahon, Joliet, and Lockport.

- Conduct in-home nursing assessments and ongoing patient evaluations
- Perform initial start-of-care and re-certification visits as needed
- Accurately complete OASIS documentation
- Provide Skilled nursing care including wound care, IV line care, and maintenance
- Educate patients and caregivers on disease processes, medications, and plans of care
- Monitor patient progress and promptly report changes to physicians
- Coordinate care with therapists, HHAs, and other disciplines
- Supervise HHA field staff as appropriate
- Ensure compliance with Medicare, agency policies, and clinical standards
- Complete documentation and paperwork in a timely, efficient manner
- Active Registered Nurse (RN) License
- Home Health experience preferred, but not required
- OASIS Documentation experience required
- Strong assessment, organization, and documentation skills
- Strong communication and documentation skills
- Ability to work independently in a home-based setting
- professional, dependable, and patient-focused approach
- Physical ability to perform in-home visits
