We are seeking a Part-Time/PRN Physical Therapist Assistant (PTA) to join our home health team serving the Porter County, IN area. This role is ideal for PTAs seeking flexible scheduling, supplemental income, or a healthier work-life balance while delivering meaningful, hands-on care to patients in their homes.

- Provide in-home physical therapy treatments under the supervising PT's plan of care
- Teach patients and caregivers proper exercises, mobility techniques, and safety strategies
- Monitor and document patient progress accurately and timely
- Communicate updates to the supervising Physical Therapist and interdisciplinary team
- Deliver care in accordance with state, federal, and professional standards
- Promote patient safety and maintain compliance with agency policies
- Participate in interdisciplinary collaboration as needed
- Licensed Physical Therapist Assistant (PTA) in the state of Indiana
- Graduate of an accredited PTA program (APTA/CAPTE)
- Minimum 6 months PTA experience (home health preferred, not required)
- Strong communication and documentation skills
- Reliable transportation, valid driver’s license, and required auto insurance
- Compassionate, dependable, and patient-focused approach
